Warnings that could have prevented one of Britain's worst civil engineering disasters were ignored, according to a new safety report.
The Health and Safety Executive has criticised project managers responsible for three tunnels for the express rail link service at Heathrow Airport which collapsed in 199. The report follows detailed investigations and legal action against the firms involved: Balfour Beatty Civil Engineering and Austrian firm Geoconsult.
The report accuses the companies of cost-cutting, poor design, poor workmanship and lack of supervision and safety management.
Kevin Myers, the HSE's chief inspector of construction, said human errors were a result of "foreseeable organisational failures".
"The collapses could have been prevented but for a cultural mind set which focused attention on the apparent economies and the need for production rather than the particular risks," he said..
